Referral Information
If you wish to see Dr. Evans for a MSP covered procedure, please have your family doctor or any primary care provider fax us a referral. Referrals are triaged as urgent, semi-urgent, and routine based on the information provided by your family doctor. All requests for MSP consultation or surgery require a referral from a primary care physician.
If you wish to book a private consultation for a cosmetic or non-insured procedure, please contact the office at your convenience.
